The following documents MUST be attached to your application for funding when it is submitted:
- Tax Certification Form
- Davis Bacon Certification Form
- Davis Bacon Contractors Certification Form
- Contractor Fact Sheet Form
The City of Springfield is providing the following documents for information purposes. These documents will be applicable to projects that will require compliance with the Davis-Bacon Prevailing Wage Requirements:
- Davis Bacon Wage Rates
- Employee Rights Poster (Contractors should make this available to impacted employees by posting at job-sites and/or office)
- Federal Labor Standards Provision
- Certified Payroll Reporting Form
- Record of Employee Interview (This form is utilized by the City of Springfield Compliance Officers when interviewing contractor employees)
