Comprehensive Case Management

Summary of Awarded Proposal:
For this project area the Gandara Center submitted an application which proposes to provide Comprehensive Case Management (CCM), by assisting individuals to navigate the landscape of existing providers, develop a plan for appropriate services, and establish linkages and relationships with corresponding agencies to provide intensive wraparound supports for approximately 3-6 months.
The CCM will be delivered by bilingual/bicultural staff who have lived experience with SUD; to PWLLE individuals, and their families, impacted by OUD and multiple other high-risk factors with significant unmet health related social needs, who reside within the City of Springfield. The CCM will be framed around screening to identify an individual’s medical, psychosocial, behavioral and functional needs, and then working directly and/or through community resources to address these needs while the OUD is treated. The CCM will be grounded in an understanding of clients’ experiences and the world they inhabit-the nature of addiction and the problems it causes, and other problems with which clients struggle (i.e., co-occurring disorder or incarceration). Culturally sensitive practices reflecting diversity, race, gender, ethnicity, disability, sexual orientation and life stage will be built into the case management process. The program will utilize a pool of emergency flex funds to further minimize barriers to accessing and maintaining services.
The CCM will aim to enhance access to essential services and support; reduce barriers to treatment and recovery; and decrease the number of individuals who reengage with law enforcement, and emergency medical services, as measured by quantity indicators (number of persons served), and quality indicators (of those served, number who return to police involvement; number who experience a subsequent non-fatal overdose or OUD related medical incident, and number who accept, receive and maintain services recommended).
Gandara will appoint a Program Director (PD) to implement and provide oversight for the proposed project. The PD will be overseen and supported by Gandara’s Vice President of Behavioral Health and Recovery Services. The Program Director is responsible for: 1. Maintaining daily oversight and supervision of the program and case managers, 2. Primary point of contact for partner referrals (HCSD, BHN, HUB Access Navigators), 3. Participate in SPD’s HUB “situation table”. Three (3) Comprehensive Case Managers will be overseen and supervised by the PD, who work as an active part of a consumers’ care team. The Comprehensive Case Managers will provide: 1. assessment and service planning to address OUD and identified Social Determinants, 2. wraparound case management and serve as the primary point of contact for persons served and all other service providers involved in their care, 3. intensive services for 3-6 months (longer as needed).
