Outreach Division
Let our Outreach Division Help!
We offer the following services to anyone 60 years of age and older. Our ultimate goal is to keep individuals as independent as possible while living in the community. We provide referrals to various community resources when necessary and advocate for those in need. We provide assistance with completing housing applications, applying for discounts offered by the City of Springfield, applying for Supplemental Nutrition Assistance Program (SNAP), fuel assistance applications, and a wide range of additional services. We are supported through the Massachusetts Executive Office of Aging & Independence (AGE), Massachusetts Council on Aging (MCOA), and the Federal Administration for Community Living (ACL).
Monthly Programming
Our division helps to educate families and individuals on topics such as medication interactions, Alzheimer's disease, diabetes disease management, grief and loss, estate planning, and many other educational topics. All events and education workshops are held at one of our senior centers: Clodo Concepcion Community Senior Center at Greenleaf, North End Senior Center, Hungry Hill Senior Center, Raymond A. Jordan Center. Most programs are free and offered during the day, but some programs are scheduled in the evening as well.
