Outreach Division

Let our Outreach Division Help!

We offer the following services for anyone 60 years of age and older living in the City of Springfield. Our ultimate goal is to keep individuals as independent as possible while living at home or with loved ones. We provide case referrals when necessary and advocate for those in need. Connections to in-home services. We also connect individuals to physical and mental health providers, assisting in the housing application. Supplemental Nutrition Assistance Program(SNAP), fuel assistance applications, referrals for meal programs such as meals on Wheels, and a wide range of additional referral services. We are supported through the Massachusetts Executive Office of Elder Affairs, Greater Springfield Senior Services, Inc. (GSSSI), and the Federal Administration for Community Living (ACL)

Monthly Programming

Our division helps to educate families and individuals on topics such as medication interactions, Alzheimer's disease, diabetes disease management, grief and loss, estate planning, and many other educational topics. All events and education workshops are held at one of our senior centers: Clodo Conception, North End Senior Center, Hungry Hill Senior Center, Raymond A. Jordan Center.  Most programs are free and offered during the day, but some programs are scheduled in the evening as well.

Brown Bag Distributions: Food for Elders program provides a free bag of healthy groceries to eligible seniors once a month.
The food items are specifically selected to meet seniors’ special dietary requirements to help them stay healthy and active.

Eligibility: The Brown Bag program is for people over the age of 55 who meet certain Income Eligibility. If you qualify for SSI, Medicaid, Fuel Assistance, Veteran’s Aid or have a documented disability for which you receive SSDI, you may qualify to receive Brown Bag monthly groceries.
