The City of Springfield, Massachusetts seeks proposals from responsible and responsive firms to provide Dental and Vision coverage services in accordance with vision and dental plans as outlined herein. Plans provide vision and dental coverage to employees and retirees of the City of Springfield in accordance with Massachusetts General Laws Chapter 32 (B).

SUBMISSION PROCESS

Proposals must be received in the Office of Procurement on or before July 21, 2014 by 2:00 p.m. EST. All information in the proposal should be organized and presented as directed below. The proposal should provide a straightforward and concise description of the Bidder's commitment and ability to perform services as described in this bid. Price proposals should be submitted in separate sealed envelopes.

Purpose and Objective

The City, acting on behalf of the Mayor, seeks competitive proposals from licensed healthcare providers to provide Dental and Vision coverage on an as-is basis. Additionally, the City is seeking alternative proposals based on varying levels of coverage.

Proposals are sought for the current period (FY 2015) running from date of implementation through June 30, 2015, for FY 16 running from July 1, 2015 through June 30, 2016, and FY 17 running from July 1, 2016 through June 30, 2017.

Current Funding

The City's Dental plan is fully insured - pooled as of July 1, 2013. The City's Vision plan is self-insured. The City is seeking fully insured quotes for both Dental and Vision; but, will entertain self-insured proposals as well.

If you are quoting self - insured:

1. Do you require banking arrangements for ASO funding?
2. Do you require a balance on the account or ACH debits?
3. What is the required float on claims?

Demonstrated Customer Service Results

The City of Springfield wants to ensure that participants/members are serviced appropriately. Please provide the following information for all service centers, claim administration, member services, provider services that will directly service the members/participants of the City of Springfield under this proposal.
